Patients and Families

Florida Hospital Memorial Home Health specializes in providing services in your home.  All services are provided under the direction of a physician and are planned and coordinated according to each patient’s individual needs.   Our goal is to teach you ways to manage on your own to prevent rehospitalization and relapses by learning your body and the best ways to manage your disease process.

We encourage participation from you, or if you are unable, your caregiver or family member in the planning and revision of your plan of care.
